Many people may not be familiar with this, but this is a military record document for those who served in the former army or navy during the Pacific War. It records the date of enlistment, the unit, rank, place of posting, illness, rewards, punishments, etc., and is created based on military records.
In principle, records of military records for those who served in the former navy are kept by the Ministry of Health, Labor and Welfare, and records of those who served in the former army are kept by the prefectural office that has jurisdiction over the applicant’s domicile at the time of the end of the war. In principle, most government offices can obtain a certificate for anyone within the third degree of kinship, but a family register or other document that shows the connection to the applicant is required.
However, depending on the time of enlistment in the military, such as enlistment just before the end of the war, there are cases where the war ended without a record being created. In addition, records may have been lost in fires or in the chaos after the war.

However, some prefectures will not accept applications made on your behalf, so you may only be able to collect the family register and obtain the application documents. Furthermore, if there is someone still alive who is closer to the generation of the person whose military record you want to certify, some places will only accept the application from that person. (For example, if you want your grandfather’s military record, and your father is still alive, it may be necessary for him to apply.)
